TRT Beginner Through Doc, Advice Needed

Would be Eternally grateful if the TRT Gods could guide me back to my full potential mentally!

My Name is Donny - 24 Yrs - Weigh - 220lbs Height - 6ft
Eat Clean, Train 4-5 times a week minimum. used to be obese. No Alcohol/Drugs.

I Started TRT back in August after blaming every other marker under the sun for my fatigue ,depression and low libido symptoms. From not giving up we worked out low t could be a major factor and i got a prescription from a Doctor who prescribed me TestoGel.

After 2 months i Changed from Testogel to Test Enanthate. Starting at 150mg per week. The first dose i did in one shot and left for the rest of the week but for sure this did not work. I then split the dose into two smaller doses (Mon/Thurs) and this helped fluctuations but was not feeling energised or ready to go as i had before. After speaking with my Doc he recommended i increase to 200mg per week divided between two doses and get a blood test in a couple of weeks.

Me being inexperienced i thought it was ok to take the injection an hour and 30 mins before my blood test and not expect it to skew the results in any way, ofcourse when you see the results this is exactly the case i think.

As mentioned earlier some readings may be inaccurate due to the previous injection but i’m not sure.

200mg has been the best i am feeling during this whole time but there are still some teething problems:

Current Struggles
Sleeping - Always suffered from Insomnia
Struggles to wake up in the mornings, Don’t feel like i have had enough rest, losing the ‘Go’ within myself, Deceases motivation.
Face Goes very red randomly throughout the day.
Feel warm when others are cold. (Can be Sweaty)
Anxiety like feelings, heart beating feeling and heavy breathing.

Real objectives from TRT were to mentally reframe my mind/body to grow the confidence, motivation and hunger within myself to achieve my goals and reduce my risks of any long term physical health conditions. Just want to ensure I’ve made the right decision going 5-6 months in. Any advice or observations would be really appreciated.

The problem is your have low SHBG and you’re on the wrong type of protocol, low SHBG men struggle the most on TRT when not injection very frequently. You will always have estrogen problems injecting moderate dosages and you need very frequent dosing.

You’re also converting a hefty portion of your testosterone to estrogen and daily dosing will go a long way towards minimizing the T->E2 conversion. I’m in the same boat as you, my SHBG is low to mid teens and my estrogen is in the 70’s injecting 50mg twice weekly.

You will not need very high TT levels to have FT levels at the top of the ranges, when my TT is 450-500 ng/dL, my FT is at the top of the ranges. It took me awhile to learn that going for a high TT level always caused problems with estrogen, because my FT was well over the top of the reference range.

You want to be using 1ml 27-29 gauge Easy Touch insulin syringes for your injections.

I’d like to add that TRT has totally changed my life even though there has been teething problems.

If 0% is Rock Bottom and 100% is the Rock I’m currently 50-60% (GoldDust). A number of my negative symptoms have cleared but not all!

Currently on 0.4ml (100mg) Twice a week, are you saying that because i have low SHBG this makes it easier for the excess Test to convert to Estrogen? Due to the ‘One off’ larger dosage.

Could i Split my dose into an every 3 day or every other day protocol instead. would it have the same impact? I travel so may be hard for me to dose every day, although this is priority if its the only way to stop the problems. I also pin in my Quads as i felt i’m not experienced enough to self inject in my Glutes, I’ve been doing it for 4 months now, But would Injecting ED in Quads cause issues?

Thanks for the advice will be getting these, is this better just generally or more for ED injections?

Also System any info on SleepApnea? had it for years but has developed over the past month or so, do you think it could be influenced/effected by the above?

SHBG binds testosterone and estrogen and since your SHBG is low, you will reach high free hormone levels sooner than say someone with high SHBG, in fact you see high SHBG men with High TT, low FT and low E2. Also these large doses will spike testosterone and estrogen very high, look at your FT level, it’s many times the reference ranges.

I’m assuming you want to reach 100% (gold nuggets) and feel your best, that’s not going to happen on a twice weekly protocol or it’s not going to happen on an EOD protocol.

All these symptoms are estrogen related, by choosing twice weekly or EOD over the daily protocol you are in fact choosing to NOT feel you best. The smaller the injection, the lower the estrogen, the better you’ll feel.

This shouldn’t cause any issues in how you feel.

28-30g insuline needles won’t create as much scarring as the larger guages with more frequent injections. Also less painful. Moving to those will also allow you add in delt injections as they are usually only about 1/2" long versus the 1-1 1/2" needles most are prescribed.
