Thoughts on Low Total Test vs Normal Free Test?

Hello,
I’ll start by saying I’m 26 years old, decent shape, 6 foot, around 205 lbs. I lift intensely 3 times a week, eat decently clean, don’t smoke, don’t do drugs, and drink a few beers socially once a week. I have been suffering from low libido, Erectile Dysfuntion, including no morning wood, as well as having some trouble at the gym. I got a blood test from Dr which came back at 235 Total Testosterone, and 44 Free Testosterone. My total is below the normal range of 300, but since my free testosterone is within “normal range” my Dr doesn’t reccomend treatment. Thoughts?

You aren’t the first and won’t be the last to encounter medical ignorance, you have all the classic signs of a testosterone deficiency and labs confirming very low levels that of a 90 year old man and still your doctor fails to act. You are at high risk for cardiovascular disease and I suggest you find a specialists outside of your insurance network because your doctor is an idiot, most doctors don’t know the first thing about what constitutes normal sex hormones levels and instead fixate on lab ranges do to a complete lack of knowledge.

Truth be told your Total T levels is competing with 90 year old men at 26 years old. You need to seek private care for TRT because most doctors are idiots when it comes to TRT, you can get a new doctor, but I expect all the doctors you have access to to be equally as incompetent.

Doctors incompetence is a worldwide problem for men seeking diagnosis and treatment of a testosterone deficiency, doctors receive no education or training in medical school for anything related sex hormones. In reality doctors are taught to treat the reference ranges and ignore symptoms and that’s exactly what your doctor is doing.

It’s not really a stretch of the imagination that ED, no morning wood and low libido is being caused by a testosterone deficiency, labs support it as levels are at the bottom of the ranges.

If your Free T is in range with testosterone very low, that would mean SHBG is very low and any TRT protocol would need to consider SHBG before commencing with TRT. It’s more than likely you will need multiple smaller injections per week to minimize side effects.

These data showed that a testosterone threshold of 440 ng/dL was associated with increased Framingham 10-year CVD risk in middle-aged and elderly men. Poor sexual performance, decreased morning erection, and loss of libido had an impact on the testosterone threshold for CVD risk. The threshold level was higher in men with sexual dysfunction.

What is the range on the free t.

Free test is 44

Ranges for Free T differ from lab company to lab company, Labcorp ranges are 6.8-26.5 for men your age. A free T of 44 suggests your lab company has different ranges and you need to provide them for all lab values.

Take this time to educate yourself because your doctors won’t be, you have to help yourself.
