XC ski fans…similar to hockey in that they PLAY the damn sport as well as follow it ; )
Swimming or sprinting
The support system for these sports and the culture around them are very strict from my experience.
I think solo sports fans are usually former athletes and very in tune family members.

I’ll watch international tournaments (ie: our crap-tastic World Junior team this year), but only watch the NHL highlights. Too much of a time commitment to follow a team IMO.

No, my guess would be NASCAR has that distinction. I think most actual drivers are pretty fit. It’s apparently a very physically demanding occupation. But my guess is most of the fans consume half their bodyweight in beer, pizza, and smokeless tobacco every day. And that’s the female fans. The guys can manage more than half their bodyweight.
As for healthiest fans of any sport, I would guess it would be among those Olympic sports that are only shown online or on TV at ungodly hours like 3AM: like Olympic lifting, bike racing, and some of the track and field events. That’s because most of the people who go out of their way to watch that are also competitors themselves, or former competitors, or coaches in those sports. That’s true for golf too. I don’t think you could find anyone who watches golf but doesn’t play it, but recreational golfers are hardly a fit bunch.
Swimming because the only people who can sit through a swimming gala in its twelve hour entirety are other swimmers.
