These look good to me other than the slight forward jump.

Form started to break down on last reps. Hips are rising too fast?

all looked pretty good! keep elbows turned out in cleans (and snatches), bar close; this should straighten the path a bit in second pull/you’re looping the bar round a touch. re clean dl, focus on keeping shoulders over from start and stay over in second pull, you’re a touch behind at points, hence hip movement. there’s a great article by jim schmitz on iron mind re clean dl.
