You should inject twice a week for steadier levels and then lab results would be more representative of whats going on.
But as you seem to be a T hyper-metabolizer, you should be injecting EOD and do labs half way between.

I do think that I’m a hypermetabolizer. I had TT levels in the 500 range 4 days after a 200mg shot.
Every time I try to inject twice a week after a few weeks it becomes very painful to inject. I rotate hips, but I guess scar tissue builds up?

I’m assuming you’re harpooning yourself in the glute based on your comment on hip pain? You can inject with insulin syringes in the quads.

Yes I use a 23 gauge x 1". Once per week rotating sides has never posed an issue, but twice a week rotating sides starts to hurt after a few weeks.

You may have much better success using 29 gauge 1/2 inch syringes. Twice per week in the quad/IT band should be fine twice per week.
