Fatigue: Please post your oral body temperatures when you wake up and also mid-afternoon. If low, thyroid function is low.
Are your outer eyebrows sparse?
Do you get cold easily?
Do you use iodized salt and/or vitamins that list iodine+selenium?
How much has stress been a factor in your life?
What is the time line of your issues?
