Huge Discrepancy in my Total T vs. Free T

My total testosterone came out to 504 and my free testosterone came out to 9.4. For the past year and a half, my testicles have been gradually shrinking (I’ve been applying clotrimazole to them twice a day for jock itch for about 3 years) and are now noticeably smaller (even my girlfriend noticed it without me pointing it out to her - I asked her if my nuts had always been this way and she said no way).

In retrospect, I lost about 15 lbs in lean muscle all over my body within 6 months of applying this jock itch cream to my scrotum. About 6 months ago, I read that azole drugs can disrupt the action of the P450 enzyme and reduce testosterone and abruptly ceased using clotriamzole. I have had intermittent testicular pain since January.

I’m 27 years old and my activity levels are higher than they’ve ever been.

Please help if you can. A urologist told me my free testosterone was totally normal for my age when it obviously isn’t.

I’m going to see an endocrinologist on Tuesday, but I doubt he’ll be of any help. I feel screwed.
