Defy Medical: Dr. Saya or Dr. Crisler? Who Would You Recommend?

I’m in the new patient pipeline at Defy. I’ve been on TRT for 6 years but have never had a true hormone replacement specialist get me dialed in. I’m debating between working with Dr. Saya or Dr. Crisler. Can anyone speak to their experiences with either or both of them? I have some unique/complicated circumstances (elevated IGF-1) that I’m working on getting evaluated at the same time, so I’d prefer to work with whichever doctor there has the deepest understanding of these systems and the most expertise in addition to a good approach to complete HRT therapies.

Dr. Criser lies awake at night deep in thought about hormones, he’ll know based of what you’re telling him whether or not you experiencing high estrogen symptoms. Dr Saya is the same so they are both really, really good at what they do.

It’s nice to have a doctor be able to know what’s wrong with you before labs are even drawn.

Cool. So you think it’s a six-of-one, half a dozen of the other type situation? Can’t go wrong with either one? I’m scheduled with Saya for mid-August so I’ll see if Crisler is available sooner.

Dr Crisler used to work with Defy but I though he left quite some time ago to start his own practice in another state. Did he get back with Defy? Last time I checked it was just Dr Saya and Dr Calkins.

You’re absolutely right. I was under the mistaken impression Crisler was still with Defy. He used to have an independent practice, joined Defy, and has gone back out on his own again. I’ve already paid for my consult with Dr. Saya in August, only question is whether I do an advice-only consult with Dr. Crisler about an elevated IGF-1 level I got back on my onboarding bloodwork with Defy.

Well your first consult with Defy will be a talk about everything, diet, general health, trt that is why its so expensive compared to followups. So your IGF-1 will be included in your TRT discussion. I would wait until you get those results before laying out more money where you could possibly get the exact same answer.

I am currently with Dr. Crisler. His practice is pretty good. It just took me awhile to adjust too because you do all of your communication with his staff through email. But they are pretty good at responding in a decent amount of time. Let me ask you guys a question would it be dumb of me to join Defy and just have both doctors and do you think that will complicate things???

Why would you pay double for basicly the same advice and scripts?
If you have access to T cyp/HCG/Anastrozole what else do you need?

Real differences in the two is attitude and personality whereas Dr C is a bit disagreeable to me. I will soak up his writings but I don’t find him to be someone that I can work with where Dr Saya is completely different and suits me quite well. Though they are two of the preeminent Drs in the field they do have diverging (medical) opinions on TRT.

I do agree Dr. Crisler does have his moments to where he will disagree. So as of rite now I am with Dr.Crisler but I’m also with another doctor as well I really like having the option of having both at hand because for instance the Dr. that is close to me he practice was able to find me anastrozole compounded to .1 mg which was one of the best things that’s ever happen to me because it allowed me to really tweak my E2 to where I needed it to be. But Dr. Crisler can not do things like that he only has a variety of different medications he uses. But in the other hand Dr.Crisler is way more knowledgeable in all areas when it comes to TRT. So each doctor brings pros and cons to the table. But that why I was asking if it will be any benefit if i join with Dr.Saya …

@systemlord didn’t you say you are with defy how often are your follow up appointments ??

I’m with Defy and you can have follow ups as often as you wish, I prefer to have closer supervision in the beginning so I can optimise treatment, then once treatment is dialed in you can do 6 month follow ups which are mandatory.

I had a mini consult a week ago because I was having problems, estrogen spikes was causing blood pressure spikes. My estrogen blocker was increased and I feel better.

How do you decide when a “mini-consult” is ok to use vs a “follow up”?

The people over at Defy suggested the mini consult because I had informed them the situation was serious. I didn’t have to wait long, the very next day a plan had been put in place to deal with the side effects.

I’m going to follow up in 3 weeks.

@systemlord So maybe the mini consult isn’t used when the situation is “I still don’t feel dialed in.” But it would be used for “I have this single issue that I need fixed right now.”

I have only done my initial consult with Defy and I never found out what the difference between the 2 types was.

I don’t always want to quote the post, but want it to show up as a reply to a post. Sometimes the top right of a post it shows who is being replied to. I can’t get that to ever show up when I click reply in the post.

@systemlord I had a quick question my doctor wants to put me on a scrotum cream along with HCG and DHEA he said that the scrotum cream will create a lot of DHT. What are your thoughts on this ?

Your doctor is correct, the cream applied to the scrotum will increase DHT, if he has you on DHEA then you must have lower levels and low DHEA worsens the symptoms of low testosterone. HCG is more about fertility and a few need it or else it could make or break TRT.
