37 Year Old with Varicocele

This is my first post and not happy about this Varicocele thing. Let me begin by saying I used to hit the gym like 4-5 days a week, i was like 145 and 5ft7. One day after I had intercourse I was all good, two days later I was feeling pain in my right testicle so i immediately went to the E.R. By telling the doctor i had unprotected sex he assumed it was a bacteria affect that caused the pain so he gave me a antibiotic shot which took the pain away, he also said something about Varicocele that was on the ultrasound which he didnt speak about.

Since I was feeling ok i went back to the gym lifting heavy as usual and about a week in a half later my right testi started back aching so i went back to the E.r and did another ultrasound and this time it was only a Vericocele. The nurse said I had bilateral Varicocele, the left vein was swollen and the right one was small. The nurse told me I may need surgery and to check with their urologist. I met up with the urologist and basically check my prostate and said my right prostate was a lil swollen. So got me going to physical therapy. I had a ct scan and my pelvic area came back negative.

My semen came back normal. So I hit the urologist back up and asked him if i need surgery becouse im still in pain, he said i dont need surgery bt email. So i went for a second opinion from a different urologist like two days ago. He basically did a full check, had me lay on my back and stood up to check for Varicocele. He said i have a small Varicocele on my left side and a tiny one on the right.

He basically told me to wear a jockstrap and take Alieve medicine and said come back at the end of the month sonhe can referred me to a pain specialist who may give me a pain shot to the back. I asked the urologist about should i get surgery because its a sharp pain and it causing me to leave work early all the time, he say your Varicocele are too small. I just need some advice fellas, i been depressed and been staying home lately becouse i used to be active and now i be in pain. Can anybody give me some tips or what shall i do becouse im stuck and maybe lose my job over this Varicocele thing

Prostate inflamed and he prescribed therapy.
If you have prostatitis u require at least 30 days of antibiotics. Prostatitis can have radiating pain to testicles. Prostate would be very tender if it was infected. So when they do the finger thing it would basically hurt when they touch it. When Drs do they rectal exam they measure with their finger.

Also do a urine culture to check for any bacteria.
May also be Orchitis? Maybe. I mean they can usually tell with the ultra sound . Hopefully they did a Doppler ultra sound.

I know constant pain their is depressing. Consider my info above.

Where are you located? Odd that doctors will pass up a opportunity to make some money.

Varicocele can lead to low T levels. I can’t really help with the issues you describe or what it takes to get appropriate action. But below there is information that guys need when the problem creates unresolvable low T levels.

Get these tested: TT, FT, E2, LH/FSH

Evaluate your overall thyroid function by checking oral body temperatures as per the thyroid basics sticky. I can be a bit abrupt in my replies and recommendations. I have a lot of ground to cover as this forum has become much more active in the last two years. I can’t follow threads that go deep over time. You need to respond to all of my points and requests as soon as possible before you fall off of my radar. The worse problems are guys who ignore issues re thyroid, body temperatures, history of iodized salt. Please do not piss people off saying that lab results are normal, we need lab number and ranges.

The value that you get out of this process and forum depends on your effort and performance. The bulk of your learning is reading/studying the suggested stickies.

No. Repeat the UA. You’re getting a lot of possibilities thrown at you:

  1. varicocele
  2. prostatitis
  3. kidney/bladder stones/infection
  4. spinal injection for prostate pain?
  5. low test? why?

Repeat urinalysis is simple enough. Once that is settled deal with the testicles. Determine if the varicocele is causing low test. Apparently, the two urologists don’t think surgery is indicated. Even if the pain subsides, I’d check testicular function.

If you was in my shoes where u go for testicular function. I dont want to go back to those doctors, Sometimes i think they playing the race card on me :tired_face: its depressing

Find a TRT doctor. Read the items suggested above.

Ok I will, I just got my results for my semen last saturday which came back normal. Is that the same as testosterone, sorry bro im new to all this

I had varicocele on left testicle when I was a teen.

It only caused me “feeling heawy” sometimes.

Ultrasound showed my testicles are both big equally big and healthy.
My testosterone levels were also really high (I was 17 at that time).

I had surgery and I don’t recall having those problems since. It’s possible this will ‘return’ but even if it does - I’ll just go to another surgery and take care of it.

No, and “normal” essentially means you should be able to conceive. Sperm counts range from 40million/mL to 300million/mL. Below 15mil is considered abnormal or low. Part of normal includes the motility and viability of the sperm. Sperm are made in the testicles.

Testosterone is a steroid hormone also made in the testicles. Like sperm counts, “normal” encompasses a wide range. You could have low numbers of each and still be considered within normal ranges. Testosterone is evaluated with a blood test, not a semen sample.
