You clearly have two separate issues, your T numbers are below borderline acceptable and your thyroid is near the upper limit and then there’s the issue that these ranges aren’t narrow enough. Some doctors start thyroid treatment when TSH gets above 2.5, you don’t wait until thing get 100 percent bad, you act sooner. The problem is doctors don’t consider treatment until you reach “disease status”, they don’t optimize your health do to increases in the cost of healthcare. Very few doctors would treat your TT numbers in the UK, what did your doctor say when you told him of your experience with taking testosterone? You can’t argue with the results.

Your thyroid is in trouble. Where you every not using iodized salt in your past? - YES! [the damage may be done then]

Please go to 200mg selenium which you can get in many multi-vits. Start taking:
1000mg Vit-C
natural source Vit-E
fish oil and/or flax seed oil/meal

“handful of Brazil nuts” should be more than enough selenium, so that throws some selenium ideas out the window.

fT3 is the only active hormone which acts on mitochondria in your cells as part of your body’s temperature control loop. rT3 can oppose fT3 which then leads to higher TSH. Higher TSH levels aggravate the thyroid auto-immune state that is driven by inflammation from normally produced oxygen free radicals by product of T4 production. Selenium is needed and part of the enzymes that cleanup the free radicals. I suggested some anti-oxidants.

We really need to consider fT3 and body temperatures together. And rT3 may be a consideration.

Your CRP is not indicating major inflammation in your body and localized inflammation in your thyroid may not affect CRP.

@systemlord thanks for your feedback, it is much appreciated. I’m actually in Canada, but I think the same thing holds true here with doctors not starting treatment until reaching “disease status.” When I told my doctor about past testosterone supplementation, she just brushed it off and wanted to send me for bloodwork. I have a follow-up bloodwork appointment this Friday, and then meet with her again in the middle of January, at which time I will ask her about starting thyroid treatment.

I hadn’t been using iodized salt in the past. I did use iodized salt with every meal now, and also did a IR in June after my first round of blood work (although I’m not sure it did much if the damage has already been done).

I do not have Brazil nuts in my diet anymore, so I will start taking 200mg of selenium, as well as the other supplements that you listed.

I have follow-up bloodwork this Friday to test TT, FSH, LH, prolactin and PSA, and then I meet with my endo again in the middle of January. At that point, what should my next steps be? Test fT3? Ask her to start thyroid treatment based on my results? TRT?

I have been tracking my body temperatures over the last few months and am posting them as a reference for if/when I get my fT3 tested.

I have also started taking 200mcg of Selenium per day in a multivitamin, as well as 1000mg Vit-C, 800iu natural source Vit-E, and fish oil as @KSman has recommended. My body temperatures have gone up slightly since starting, and I haven’t felt as cold in the afternoon, although they still appear to be low. It looks like at this point, the next step will be waiting for my follow-up appointment with my endo in a month, at which point I will ask to have my fT3 and rT3 measured as well. Although my testosterone levels are below borderline acceptable, I think my blood results point to my thyroid being the main issue here.

Checking in with the last few weeks of oral body temperatures. They’re obviously still low, even after starting on the supplements recommended by @KSman. I have ordered some more Iodoral to do another IR, and this time after I finish I plan on supplementing with 6.5mg/week as a maintenance dose.

Updating my oral body temperatures, as well as noting a couple other things. I am currently on day 11 of my IR and initially my temperatures got lower instead of higher. I thought that this may be due to the iodine causing transient TSH levels, as Dr. Chris Kesser has noted, “be aware that high doses of iodine can lead to a transient increase in TSH levels, which can be mistakenly interpreted as a sign of hypothyroidism.” Of course, I am doing the IR with 200mcg of selenium, as well as the other recommended supplements. They have since increased but are still not at the levels recommended by @KSman. I meet with my endo again on Jan. 17th and will be asking to get my fT3 measured, as well as start looking at possible thyroid medications. I’m hoping that @KSman can jump in here and recommend which would be best given my situation.

So I finally had my follow-up appointment with my endo today and she first went over my testosterone numbers. She said that they were completely normal and nothing to be concerned about at all. I mentioned that I interpreted them to be below borderline acceptable as @systemlord had pointed out, however, she said that the lab ranges are adjusted for sex and age? I’ve never heard of that before, but just let it go…for now.

As for my thyroid, she was concerned with my results, and stated that I have early onset Hashimoto’s disease. She said that hypothyroidism typically has an insidious onset with subtle signs and symptoms that may progress to more advanced or even florid signs and symptoms over months to years, and that I could wait until the symptoms become more severe, or could start with thyroid medication right away. I obviously opted for the latter and now have a prescription for Eltroxin at an initial dose of 50mcg/day. She is also sending me for an ultrasound, and I have follow-up bloodwork in April to see if this dosage needs to be upped.

Although I am not 100% satisfied with the outcome today, I am optimistic and feel like I am finally making progress in getting all of this solved. The thing that I question the most is that most guys that I see on here that have Hashimoto’s disease, also suffer from low testosterone as a result. I still feel like more work/investigation needs to be done into my testosterone levels before I am able to write them off as being “normal” as my endo says.

Any thoughts on all of this @KSman? I believe I read somewhere that you only take thyroid medication for T3(?), so I’d be interested in hearing what you think about what my endo has started me on.

I had my ultrasound done today on my thyroid and it confirmed that I indeed have thyroiditis, mostly likely as a result of Hashimoto’s disease as indicated by my high levels of Thyroglobulin Antibody and Thyroid Peroxidase Antibody. Looking back on this journey, I am afraid that doing an IR may have expedited or even caused this autoimmune dysfunction. I know that @KSman is a well-respected member of this community, but I would strongly recommend against anyone does an IR before speaking with a medical professional. Frankly, the information in this article (https://thyroidpharmacist.com/articles/iodine-hashimotos/) on high-dose iodine and the thyroid is scary, and I wholeheartedly regret doing an IR. My TSH before ever doing one was 2.0, and after doing IR, it shot up to 3.86 on my next round of bloodwork. So I urge you to consult a medical professional and DO NOT DO IR, as I do not want you to end up doing irreparable damage like I may have done by doing so.

The objective is low temperature near when you wake up and peak mid-afternoon. Not evening.

I don’t get up most mornings until 10:30-11:00 am, so I thought that I may need to push my mid-afternoon temperature readings back until early evening. Getting up earlier now, as work has changed, so will start taking mid-afternoon temperature around 3:00 pm each day.

I have actually just started on Eltroxin, so I am curious to see if this affects my oral body temperatures or not.

@KSman are you able to speak to the other issues that I have posted recently? i.e., starting on Eltroxin, high dose iodine causing my Hashimoto’s, etc.

High dose IR will increase TSH at that point in time and the only harm in that is making the TSH results of no interpretive value.

Selenium is protective and prevents free radical damage created during thyroid hormone production. The enzymes that clean up the free radicals need selenium as the enzyme catalytic reaction sites in the enzyme structure.

I have stated this need hundreds of times. Nodules are created from high TSH+time. Auto immune is caused by the inflammation from free radicals where the the immune system cleans up damaged cells and misinterprets the mess a foreign and in error creates antibodies that then attack thyroid cells and one is autoimmune.

T4 only meds work well for those who are able to convert T4–>T3 in what is left of the thyroid and importantly in peripheral tissues [rest of body]. Some are not good at that and with T4 can be T3 deficient and have hypo symptoms and maybe worst off. Many docs do not understand because the drug reps do not want to detract from there T4 sales and most drug reps only know what the companies want them to know. In my case, Nature Thyroid and Armour Thyroid did not restore my body temperatures, time release T3 did. Time release T3 in USA is a compounding pharmacy only product.

Many have turned around their quality of life with iodine and IR. There doctors were not concerned with their symptoms as their thyroid numbers were all normal and I have only seen one instance where a doctor has asked about iodized salt use. I have seen people put on low sodium diets by doctors with thyroid problems created by the doctor’s ignorance or apathy. I have also seen doctors put guys on thyroid meds when the the problem was an iodine deficiency, when the the thyroid deficiency was a root cause and not addressed.

I did not make up these IR suggestions, check the WWW and there are some doctors who advocate for this as well, but perhaps they are extreme.

I am well aware of the concerns created years ago when iodine was introduced to deficient populations and hashi increased. Part of that was increased hashi as teams were now studying the population and catching problems that would not have been diagnosed before. Some hashi cases were latent and pre-existing but quite because thyroid activity was throttled by low iodine. And if the population was also selenium deficient, not understood back then, free radical damage could trigger hashi and perhaps all of this is worse in the nodules which were created by long term iodine deficiencies.

When I find a guy with low body temperature who has not been using iodized salt, do you now want me to press that thyroid anti-bodies should be tested first and if positive to be told to never use iodized salt or vitamins that list 150mcg? Where is the balance against general ignorance about the need for iodine and iodized salt?

With your prescribe T4, take note of increased energy and clarity of thought. Monitor your body temperatures and mid-afternoon 98.6F would be your goal. You may need to dose to depress TSH well below 1.0 to get there. Some docs think that TSH=1.0 is the outcome and ignore how the patient is really functioning. If you cannot get body temps up, then fT3 really needs to be tested. fT3 should be ~midrange. If fT3=~midrange and body temps are low and you feel hypo, then we have a strong case that rT3 may be blocking fT3. See comments re that in the sticky and stress issues.

There are some docs who are hostile to fT3 issues and body temps. Their world is T4 centric. I need to point out that no one has identified a T4 receptor, fT3 is the only active hormone.

There is a doctor who has claimed to calm down and reverse some hashi cases with iodine+selenium, stressing two weeks of selenium first.

@KSman, thank you very much for the well thought out, and detailed response. Today has been a roller coaster of emotions after being told by the ultrasound technician that my thyroid does not look good, and thinking that IR was the cause of my Hashimoto’s thyroiditis. I apologize if I came across snide and reactionary.

I will continue to monitor my body temperatures, making sure to measure them when I first wake up, and around 3:00 pm everyday. I often work outside and wonder if the Canadian winter negatively affects my body temperature in the mid-afternoon?

I actually brought up the issue of body temperature and the consideration of fT3 with my endo as you recommended, and she agreed test it on my next round of bloodwork in the first week of April. We will be testing TSH, Creatinine (eGFR), Vitamin B12, Ferritin, CBC, fT4, fT3, TPOAb, and Antithyroglobulin antibodies.

Interesting to hear that the natural desiccated thyroid meds didn’t restore your body temperatures. This was actually my preference over starting Eltroxin, but my endo didn’t agree. I am now curious to know if any pharmacies here in Canada could compound a time released T3 medication…

Checking in with my recent body temperatures. I started on Eltroxin (Levothyroxine) on January 18th, and haven’t noticed any improvement in my body temperatures. I never felt fatigued or noticed any symptoms of hypothyroidism before, so I can’t really comment on that either. I’m almost feeling like there was really no reason for me to start on T4 medication. I’ll be interested to see my blood results when I go for labs again in the first week of April. At that point I will most likely ask my endo to switch me over to NDT.

I have also decided to try an HPTA restart as per @KSman’s protocol. I am doing 250iu of HCG EOD with 0.5mg of anastrozole/week in EOD dosing. I added 5ml of vodka to 5 anastrozole tablets and worked it out to be about 35 drops per ML, which equates to 5 drops EOD on the same day that I inject the HCG. After the first couple of shots, I actually felt the opposite effects of what you should be feeling. My penis became small and shriveled, which told me that my estrogen was either too high or too low. Has anyone experienced this during a restart before? I laid off the anastrozole for the last couple of days, and things seemed to improve quite a bit. I was experiencing morning erections and things hung normally down there again. I did take another 5 drops with my HCG shot today, but my question is…is the anastrozole absolutely necessary while doing the 250iu of HCG EOD? Any input would be greatly appreciated.

Also, I’m thinking about getting back on Ashwaganda, as I’ve read that this has helped people with Hashimoto’s. Any thoughts on this?

I really would not start anastrozole unless u need it. Only way to tell is the estradiol sensitive blood test. Everyones e2 levels is different in response to HCG or T. A lot of guys start anastro right away and crash their e2.

I did not read up but what were your TSH, Ft3 and ft4?

Thanks for the feedback @anon10230041. I continued with the anastrozole and haven’t experienced any kind of shrinkage since that first week. In fact, it seems as though my body has adjusted and my soldier is now larger and hanging more flaccidly. Strange, huh?

I have since started Eltroxin (Levothyroxine) and my next round of blood work will be the first week of April to see the effects that it has had and will measure TSH, Creatinine (eGFR), Vitamin B12, Ferritin, CBC, fT4, fT3, TPOAb, and AntithyroglobulinAB. I may also try and sneak rT3 on there as well, lol.

Checking in with my recent oral body temperatures. I’ve been on Eltroxin (Levothyroxine) for just over a month now, and am also 3 1/2 weeks into my HPTA restart. I’m noticing that I’m now experiencing nocturnal and morning erections occasionally, and that my penis and testicles are hanging more freely as of late. I’m still having issues with ED and am using Cialis to help with this, although I would like to eventually stop using it completely in the future. The one issue that I had recently I attribute to “stim dick,” as I took a preworkout that contains DMHA and Eria Jarensis, and this always causes my penis to turtle into my body. @KSman, any thoughts on these recent body temperatures since starting Eltroxin? I suspect that a lot of the time, my lower body temperature in the afternoon is the result in living in Canada and working in frigid winter weather, as my job involved a lot of walking outside. I did also start supplementing with Ashwaganda, as I have been reading that this can help people with Hashimoto’s.

Checking in with my latest body temperatures. Judging by my afternoon temperatures, the 50mg of Levothyroxine isn’t doing much for me in terms of improving my symptoms. When I visit my endo in mid-April for a follow up appointment, I will definitely be asking her to switch to NDT so that I am getting the entire spectrum of thyroid hormones. I am also just over 7 weeks into my HPTA restart. I have finished with 6 weeks of HCG at 250iu EOD and have just also finished the first week of Nolvadex at 20mg EOD. I have also been running anastrozole throughout at 0.5mg/week in EOD dosing. I’m not really noticing much at this point in terms of improvement of sexual function. I do notice that I am getting more nocturnal erections, however I am still having to rely on ancillaries such as cialis. I am wondering if this problem may not solve itself until I have my thyroid in check? @KSman any thoughts on this?
