24, Need Help with Bloodwork

Hey everyone I have been suffering from brain fog, anxiety, sex drive that has been going down past 2 years to really low now where I would rather sleep and that is bringing upon performance anxiety issues of its own and just really low motivation majority of days. I am about 6 foot 205 pounds 6 years of working out and now just seem like I store more fat tough to recover from moderate workouts I got 2 blood tests done doc is willing to put me on trt but I want other peoples opinion if I need it. All of this has caused a great deal of anxiety/relationship problems

1st test

Total Testosterone : 345 ng/dL (249-836.0 ng/dL)
free test: 8.94 pg/mL (8.80-27.00 pg/mL)
Vitamin D 75 ng/mL (32-100 ng/mL)
TSH: 1.920 uIU/mL (.270-4.20 uIU/mL)
(T4): 7.6 ug/dL ( 4.5-12.0 ug/dL)
(FT4): 1.20 ng/dL (.83-1.62 ng/dL)

2nd test

TOTAL test: 315 ng/dL (250-1100 ng/dL)
FREE test: 64 pg/ML (46-224 pg/mL)
BIOAVAILABLE test: 134.6 ng/dL (110-575 ng/dL)
SHBG: 17 nmol/L (10-50 nmol/L)
LH: 6.0 mIU/mL (1.5-9.3 mIU/mL)
PROLACTIN: 18.2 ng/mL (ng/mL) did another one and that was 10 ng/mLwith 18 ng/mL being the upper range

Both tests were done around 10am, prolactin was high for this test so a Pitutary MRI was done which came back negative and another prolactin test to see if it would come back high and came back at 10 ng/mL. I sleep pretty good about 6-7 hours a night and just wondering what could have happened in the past couple years to make me feel like this, and no AAS use either. These numbers look like I am primary so don’t think a restart would do anything. Any help would be greatly appreciated.

Sounds like you have done your homework and know what to look for.

Any sex or masturbation close to that 18 test on the prolactin?

You have very adequate LH response, which I agree does not seem to be manifesting itself with higher T.

I used to recommend guys investigate DHEA–getting it tested and supplementing with it if it was low, but I honestly didn’t see any guys respond to that.

Have you tried testing your E2 to see if it is high? You may be able to lower it and see a corresponding increase in T (especially Free T).

Outside of that and testing cortisol to pursue some sort of adrenal deficiency that is causing your T to also be low, I can’t think of any recommendations for you mate.

My E2 was 28 with high range being 40 I think that was in line I forget the units Dhea was mid range as well that was fine so tough to get to the bottom of this maybe my gonads just don’t like to produce much T anymore and just need to get on TRT

Yes that would be the most clear cut answer, but at 24, this is not common…we have seen many cases like this here though…I guess we are predisposed to getting the hard to treat guys, and my mind is just drawing conclusions that aren’t there, but yeah, as much as I hate to resign a 24 year old to a lifetime treatment, if there aren’t other options there aren’t other options…

Unless you can afford to get in with the top hormone specialists, and be patient while they try to get you back up to speed, there doesn’t seem to be much you can do other than TRT.

Seems like my ft3 could be higher?

You need to look at the potentially underlying pathology of the problem rather then having medical Dr treat the symptoms which is just using a bandaid effect which I see fail a lot of times. There are many mechanism which drive libido hormones is just 1/3 or less of them. You have neurotransmitter, genetic, thyroid and adrenals as well as gut integrity which drives all three. Need to look at hidden stressors which can be found in 7 areas, environment, structure, biological, neurological, psychological, lifestyle, and now genetics. Can guarrante one will be able to find something in one of those areas. Proper past medical history and background is crucial in order to come up with some logical piece to explain things. Point being look out side the box because when you have a person sitting in front of you in your office, what they tell you online and what is in person are usually 2 seperate things …
