22 Y/O Male - Next Steps


-22 years old
-Lifting for four years
-5’7, 173, 12% bf

Symptoms: marked decrease in quality of erections, loss of morning erections in the past couple of months, loss of erections during sex, mood swings in the afternoons (feel low energy and sad for a couple of hours for no reason, feel better, then feel bad again later that evening), difficulty focusing

Results of morning fasted blood panel from Monday:
Serum testosterone 393 ng/dL
Free testosterone (direct) 14.1 pg/mL

TSH 3.980 uIU/mL
T4, Free (direct) 1.4 ng/dL

Vitamin D, 25-Hydroxy 32.1 ng/mL

Doctor acknowledged that my testosterone was in the low end of the normal range, but did not recommend TRT. He told me the ED stuff was probably psychological. I’m coming here for a second opinion. What next steps should I take?

Thyroid lab ranges are misleading:
TSH should be closer to 1.0, 3.98 is a problem
T3, T4, fT3, fT4 should be mid range or a bit higher.
Please post with lab ranges.

What else was tested? Need numbers with ranges.

TSH can be elevated from iodine deficiency or a thyroid pathology.
But your doc thinks that you are health?

What is your history of using iodized salt and/or vitamins that list iodine+selenium?
Do you feel cold easily?

Read the label on the salt container.
Sea salt does not contain iodine unless it stated iodized.
Rock salts and pink salts are bad too.
Whole house can be affected.

Low thyroid function can lead to fat gain and inability to loose fat.
Also robs your energy.

Check overall thyroid function by checking your oral body temperatures:

  • when you first wake up, should be 97.7 - 97.8, higher is OK, 97.3 is a problem
  • also check for 98.6 mid-afternoon

The above request is not trivial and thyroid problems sometimes affect T levels.

prolactin !!!
fasting glucose
fasting cholesterol - can be too low
DHEA-S - should be high at your age
